Not sure if how substantively these forms have changed, but HUD has submitted to OMB an information collection proposal titled Informed Consumer Choice Notice and Application for FHA Insured Mortgage.
OMB has their complete background on this request HERE, AND the FHA forms themselves can be downloaded HERE. (I noticed forms includes a sample form where lender is required to compare FHA mortgage insurance costs with conventional mortgage). Any comments are due 30 days after Federal Register publication, which is scheduled for tomorrow.
